Communication Technology: Forms

I learnt how to build forms a while back, and I copied this to some success.

Rather annoyingly though, the boxes didn't line up so I had to find out how to align them. The code was rather different and included a bit of CSS so I copied and pasted it, then edited it to produce the submission form on the right.
